1. Complete the Personal Checking Account Application by filling in the Owner's Information section, and if applicable, the Co-Owner's Information section.
2. Print the Application.
3. Sign and date the Application.
4. Return the Application to your local Cape Cod Five branch (Please do not email your Application).

As soon as you return your application a Switch Specialist will personally help you transfer your existing checking account into a Cape Cod Five account that fits your specific needs.
