Educational Mini-Grants

Thank you for your interest in a Cape Cod 5 Educational Mini-Grant! The program, which is now in its 20th year, has awarded more than 1,000 mini-grants totaling over $450,000.

Please note that you will not be able to save progress on your application for support. We recommend that you review all of the questions in advance and prepare your supporting documentation prior to beginning the application.

Terms & Conditions

  1. I accept responsibility for the accuracy of the information provided on this application.
  2. I understand that if awarded a mini-grant for this project:
    • Funds will be awarded to my school for the purpose of funding this project.
    • I will mention Cape Cod Five Cents Savings Bank Charitable Foundation Trust (herein referred to as "the Foundation") as a sponsor in any printed or published materials and at events related to my project.
  3. I hereby consent and authorize The Cape Cod Five Cents Savings Bank, its agents, employees, licensees and/or assigns (herein referred to as “the Bank”) and/or the Foundation to take photographs or video of me; or to produce other types of media productions that capture my name, voice, testimonial, and/or image (herein referred to as “the Materials”) and authorize the Bank and/or the Foundation to use, reuse, copy, edit, abridge, publish, display, exhibit, reproduce, copyright and distribute the Materials for Bank and/or Foundation purposes without notifying me.
  4. I agree that the Bank and/or Foundation may identify me by name and other identifying information such as job title. I also release the Bank and/or the Foundation from all liability for claims and demands arising out of its proper use of the Materials.
